How do I get a copy of my Medical Records?

After every visit, our office will provide your records to you. We offer multiple HIPAA-compliant options for patients to receive their records. In addition, Advanced Pain Medical Group has partnered with Sharecare to ensure the accurate and timely completion of all medical record requests.

Patients

Requires email + government-issued ID. Only the patient/guardian/authorized rep may sign. A representative must provide an Advance Directive/POA/or Conservatorship.

Third Party Requests

If you are a physician, hospital, or another type of third-party requestor, please use the link provided below to request medical records on behalf of the patient.

Sharecare Gateway

If you do not already have a Sharecare account, please use this link to create one. Your account will allow you to track requests, download records, and securely pay invoices.

Medical Record FAQs

When will my request be ready?

After every visit, our office will provide your records within 72 hours.

Patients can get their medical records from their online portal at any time.

For repeat requests requiring complete medical records, please use the Sharecare site and it will be ready in 5–7 business days from receipt of your electronic request.
